Hourly Pay Range (Minimum - Midpoint - Maximum):
$47.060 - $60.730 - $68.020
The stated pay scale reflects the range that Sharp reasonably expects to pay for this position. The actual pay rate and pay grade for this position will be dependent on a variety of factors, including an applicant’s years of experience, unique skills and abilities, education, alignment with similar internal candidates, marketplace factors, other requirements for the position, and employer business practices.
What You Will Do To provide assessment, consultation, and case management for Sharp Health Plan members with high risk and complex dual medical and behavioral diagnoses. To facilitate the integration and continuity of care for medical and behavioral components of members care through effective communication. To provide seamless care coordination through effective navigation, assisting members with understanding and accessing BH benefits, available programs, and SHC system and community resources. To mobilize members and families' personal and environmental resources to receive maximum benefits for both medical and behavioral health care and achieve the fullest personal and family functioning, and optimally enhance overall quality of life.
Required Qualifications
Master's Degree in Social Work.
Behavioral Health experience.
California BBS Licensed Clinical Social Worker (LCSW) - CA Board of Behavioral Sciences -REQUIRED
Essential Functions
Assess the psychosocial needs
Completes psychosocial assessments.
Identifies the key clinical issues such as grief and loss, adjustment to illness, substance abuse, psycho-social behavioral response, psychiatric concerns with attention paid to the developmental milestones and life transitions that can impact functioning. Identify key issues related to social determinants of health.
Communication Documents assessment findings per department guidelines. Consults with PCP, BH providers and other multidisciplinary team members in development of care plan. Maintains ongoing communication with multi-disciplinary team members regarding member status and follow-up. Documents ongoing progress with members/families as required by policy and regulatory agencies, including NCQA requirements. Attends case management review meetings. Documents consultations, recommendations, and interventions and case. Identifies and communicates needs for member care conferences. Arranges / participates in patient care conference for difficult/high-risk psychosocial cases to enhance resolution of identified issues.
Community resource Utilizes and refers to Sharp Healthcare, hospital, and community resources that can assist members. Facilitates the timely transition of members between every point on the care continuum, ER, inpatient (medical/behavioral), outpatient care, alternative living provisions, B&C or SNF. Provides referrals to members and families for ongoing community services and support. Consultation and support Consults and intervenes in crisis situations. Assists staff in de-escalating volatile situations. Educates and consults with other staff in handling psychosocial crises situations.
Member advocate Encourages member and family participation in the decision-making process. Reports all mandatory protective concerns such as Elder Abuse, Child Abuse and Domestic Violence as required by law. Adheres to all mandatory and regulatory requirements specific to Behavioral Health management of communication, member information (HIPPA) and protection of specific conditions/diagnosis. Protects member privacy by maintaining confidentiality. Keeps current with regulatory and legal requirements regarding member privacy and confidentiality standards.
Member goals and intervention Develops and implements care management goals based on medical and behavioral health plan of care which address identified needs. Practices appropriate boundary setting with clients. Demonstrates sensitivity to cultural and ethnic diversity. Effective at crisis intervention and problem-solving techniques and is a resource to other case management and multidisciplinary care team members. Seeks resources and team collaboration when faced with unusual or new situations.
Plan of care Evaluates member and family response to interventions. Updates and revises care plan monthly, as needed, and appropriate.
